هذا، وتشير الأدبيات المرتبطة بعلاج الأطفال إلى أن العلاج باللعب له ثلاثة وظائف أساسية تبدو مهمة في العملية العلاجية وهذه الوظائف هي:
1- اللعب نشاط تعبيري طبيعي، حيث يشير شيتهيك ( Chethik,1989) إلى لغة اللعب، فالأطفال يستخدمون اللعب للتعبير عن الأفكار والعواطف والانفعالات، فاللعب ينبثق من حياة الأطفال الداخلية لينعكس على عالمهم الخارجي. لذا فان الأطفال يستخدمون اللعب ليعبروا عن عواطفهم وخيالهم النفسي، و يعبرون عن مشاكلهم وصراعاتهم الانفعالية النفسية الداخلية.
2- اللعب نشاط تواصلي: من الضروري أن يفهم المعالج هذا التواصل لتطوير العلاقة العلاجية مع الطفل. فعلى المعالج أن يتعاطف مع الطفل ويفسر له لعبه ويساعده على الشعور بان مشاعره مقبولة (Russ, 1995)، فالتعاطف من قبل المعالج يسهل التغير في الوظائف النفسية، والاجتماعية، والبين شخصية.
3- اللعب وسيلة لخلق الاستبصار والوعي: حيث ترى النظرية السيكودينامكية بان حل الصراعات الانفعالية والخبرات الصادمة يُعد ميكانيزما أساسياُ في علاج الأطفال، فالأطفال يكررون الصراعات النمائية أو الأحداث الصادمة التي خبروها وعاشوها، وأن العديد من هذه الخبرات يتم التعبير عنها من خلال اللعب، إذ أن اللعب يُعيد ويكرر خبرات الطفل غير السارة حتى تصبح هذه الخبرات ضمن نطاق السيطرة الذاتية (Russ, 1995).

وقد أُستخدم العلاج باللعب في أشكاله المختلفة ضمن مختلف الاتجاهات النظرية (Kessler, 1988). حيث يعود تاريخ أول استخدام للعب في العلاج النفسي إلى مجموعة من المعالجين الذين يتبنون مبادئ التحليل النفسي، فقد أشار توما و روس (Tuma & Russ, 1993) اللذان راجعا الأدب المتصل بالتحليل النفسي بالتفصيل بان أساليب التحليل النفسي استخدمت من قبل هاج هيلموث (Hug-Hellmuth) وآنا فرويد (Anna freud)، وبيرلنجهام (Burlingham) والذين استخدموا اللعب كبديل للتداعي الحر، حيث كان المعالج أكثر استجابة وتشجيعا للطفل مقارنة بما يقوم به المعالج مع الراشدين في التداعي الحر.
أما كلين (Klein) فقد أشارت إلى قيمة وأهمية التواصل خلال اللعب، وقد اقترحت بان اللعب بالنسبة للطفل مثل التداعي الحر بالنسبة للراشدين، كما أنها أيدت التفسير المباشر للعمليات اللاشعورية التي يعبر عنها الطفل خلال اللعب.
وقد تطور الاتجاه التحليلي إلى الاتجاه السيكودينامي الذي يستند في التدخل العلاجي إلى أساليب ومبادئ التحليل النفسي، إلا أن العلاج يكون اقصر زمنا، واقل تكرارا (جلسة أسبوعية بدلا من 4-5 جلسات أسبوعية) إضافة إلى التركيز على الأهداف الحالية(Tuma & Russ, 1993) . ويُعد اللعب وفقاً للمنظور السيكودينامي محور العملية العلاجية، إذ يولون اهتماماُ اكبر لاستخدام اللعب بهدف نمو الطفل، مقارنة بما توصي به تقاليد التحليل النفسي.

أما الاتجاه المتمركز حول العميل، فقد طور مفاهيمه من خلال ما قدمته اكسلاين (Axline) والتي استخدمت اللعب غير الموجه، والذي يركز فيه المعالج على التواصل مع الطفل، ويقوم المعالج خلال الجلسات العلاجية بالتعاطف والتفهم لقضايا الطفل، ونادرا ما يقدم المعالج تفسيرات لسلوك الطفل ودوافعه، وبناءاً على هذا الاتجاه، فان المعالج يثق بعملية نمو الطفل، ويدفع باتجاه النمو الذاتي للطفل، ويعد هذا الاتجاه بمثابة مدخل مناسب لاستخدام اللعب في علاج الأطفال، وبخاصة للمعالجين المبتدئين من مختلف الاتجاهات النظرية.

كذلك، فقد طور أنصار النظرية المعرفية اساليبهم لتتناسب مع العلاج باللعب، فقد أشار ماكنينبوم (Meichenbaum) إلى انه يمكن تعليم الطفل السيطرة على الذات من خلال استخدام التصور والخيال، فقد استخدم اللعب كوسيلة لتعديل الأفكار. كما طور نل (Knell) أساليب معرفية سلوكية تستخدم النمذجة Modeling والتعزيز، إضافة إلى الأساليب التي استخدامها جاردنر Gardner’)) حيث استخدم رواية القصص المتبادل كوسيلة للعلاج( Knell, 1997).
